Recovery and Rehabilitation

Our Support Coordination team is readily available to be able to provide access to information, assistance, and advocacy regarding access to supports.

We coordinate with individuals and families to identify and select supports and services. We assisting with the arrangement and coordination of appropriate, necessary, and affordable support and services, while maximising individual choice and control in line with the NDIS.

Our focus is to:

Establish and maintain relationships with people who provide, or may provide, support and services
Support the individual and their family to link with and coordinate existing supports and services
Monitoring support to ensure ongoing compliance and safety
Keep up-to-date records of support that have been arranged and coordinated and liaising with stakeholders as required.
Provide mentoring and skills development and develop crisis plans for risk
Establishing informal or mainstream support when needed.
